Rule 1025 There is no privilege under this article in a proceeding brought by or on behalf of the patient to establish his competence.

If a patient goes to court to prove they are mentally or legally competent, their doctor cannot keep their medical information private during that case. The normal rule that protects private medical details between a patient and doctor does not apply here.

This makes sense because the patient is the one asking the court to look at their mental or physical condition. Since they brought up the question of their own competence, they cannot then block the court from seeing the medical records that would help answer that question.
