Rule 7.1051 Acknowledgment of receipt of Duties of Conservator

When a court appoints someone as a conservator, that person must sign and file a specific form before they can officially start their role. The form is called the Duties of Conservator and Acknowledgment of Receipt of Handbook (form GC-348).

This form shows the court that the conservator has received and read information about their responsibilities. The court will not issue the official papers giving the conservator legal authority until this signed form is on file.
