Form 24(a) Notice of Appeal from Superior Court

This form is the official paper used to tell the court that someone wants to appeal a criminal case decision from the Superior Court. Either the defendant or the prosecutor can use it to challenge a judgment, sentence, or other court decision. The person filing must fill in their name, the county, the case number, and the date of the decision they want to appeal.

The form also includes a section asking whether the defendant had a lawyer during the trial or sentencing. This information matters because it can affect how the appeal moves forward. The person filing the form signs it and identifies whether they are the defendant, the defendant's attorney, or the prosecutor.

If the case started in a lower court (called a limited jurisdiction court), this form is not the right one to use. That situation requires a different form called Superior Court Rules of Appellate Procedure Form 2.
