Form 3 Caption for Appellate Special Action in the Superior Court

When you file a special action in the Arizona Superior Court that comes from a lower court (like a municipal or justice court), your paperwork needs a specific title block at the top. This title block shows the court name, your name as the petitioner, the name of the judge or lower court as the respondent, and case numbers for both courts.

The format has two case numbers because the case exists in two courts at once. One number belongs to the Superior Court, and one belongs to the lower court where the case started. Both numbers help the courts track the paperwork.
