Rule 7003-1 Commencement of Action
(a) An adversary complaint must be accompanied by: (1) a completed summons for each named defendant; and (2) the filing fee, if required.
(b) Adversary complaints filed conventionally must also be accompanied by a completed Director's Form 1040, Adversary Proceeding Cover Sheet.
