L.B.R. 3004-1. Filing Proof of Claim by Debtor or Trustee A debtor or trustee filing a proof of claim on behalf of a creditor pursuant to 11 U.S.C. § 501(c) and Fed. R. Bankr. P. 3004 must contemporaneously file and serve a notice in substantial conformity with L.B.F. 3004-1.1, referencing the proof of claim.
