RULE 9009-1. OFFICIAL LOCAL FORMS AND OFFICIAL FORMS
(a) The forms adopted by this Court as MLBR Official Local Forms and shall be used in cases and adversary proceedings filed in this Court without alteration except as otherwise provided in the Local Rules or in that MLBR Official Local Form.
(b) The Official Forms promulgated by the Judicial Conference of the United States shall be utilizedused in cases and adversary proceedings filed in this Court under Title 11 of the United States Code. The MLBR Official Local Forms may be amended and supplemented from time to time.without alteration, except as otherwise provided in Fed. R. Bankr. P. 9009.
