LBR 7003-1 Cover Sheet When an adversary complaint is filed, the required adversary proceeding cover sheet shall be filed as an attachment to the complaint. The adversary proceeding cover sheet shall include the complete names and addresses of each plaintiff and defendant, which may be listed on an attached sheet.
