Rule 8001-1 Transmitting a Bankruptcy Matter to the District Court When filing a notice of appeal or a motion for leave to appeal, the filing party must also file with the bankruptcy court the form "District Court Bankruptcy Matter Civil Case Cover Sheet," available on the court's website.
