RULE 1005-1. CAPTION REQUIREMENTS FOR PETITIONS Every petition filed under the Code shall be captioned:
UNITED STATES BANKRUPTCY COURT FOR THE EASTERN DISTRICT OF OKLAHOMA and filed with the Clerk of the Bankruptcy Court.
