RULE 9004-2. CAPTION The caption of each pleading, proposed order or other document shall include the name of the Court, title of the case, the bankruptcy case number and the three initials of the assigned Judge and adversary number and the three initials of the assigned Judge, if any, and shall include, directly under the case number, the number of the chapter of the Code under which the case is pending. The document shall be titled so as to clearly describe the nature of the document. A pleading that fails to identify the case or adversary proceeding in which it is to be filed, may be stricken if electronically filed. It may be received and not accepted for filing by the clerk's office if submitted for filing. See also Local Rule 1015-1 for Joint Administration and Consolidation.
1.An "Amendment to" a document consists of information that modifies or supplements a document. The original document remains effective except for the amendment. An amendment to a document shall be clearly identified as "Amendment to [name of original document]."
2.An "Amended" document consists of a replacement document that entirely supersedes an original document. An amended document shall be clearly identified as "Amended [name of original document]."
