Rule 7003-1 Commencement of Adversary Proceeding
(a) Adversary Proceeding Cover Sheet Every Adversary Proceeding not electronically filed shall be accompanied by an Adversary Proceeding Cover Sheet (Official Form 1040).
(b) Summons A form summons need only be prepared by Plaintiff for issuance by the Clerk if specifically requested by the Clerk.
