1007-3 Payment Advice Affidavit Debtors who do not receive payments from employers shall file an affidavit or declaration under penalty of perjury in lieu of filing payment advices pursuant to 11 U.S.C. § 521(a)(1)(B)(iv). See Local Form 9.
