LOCAL RULE 9004-1 PAPERS - REQUIREMENTS OF FORM Form of Certificate of Service.

Whenever the Federal Rules of Civil Procedure, the Federal Rules of Bankruptcy Procedure, or these rules require proof of service, the proof of service must include the following information:

• the recipient to whom service was addressed;

• the method of delivery by which service was effectuated;

• if service was made other than electronically through the court's CM/ECF System or by hand delivery, the address to which service was sent; and • the identity of the person who effectuated service.
