Rule 9013-3 Certification of Service.
Each document filed with the Court, except for those served through the Court's electronic filing system, must include a certificate of service containing the following information:
(a) the title of the paper served;
(b) the names and addresses of all persons upon whom the paper has been served;
(c) the entities the persons served represent; and
(d) a description of when and how service was made.
