RULE 3004-1. FILING PROOF OF CLAIM ON BEHALF OF A CREDITOR If a debtor files a proof of claim on behalf of a creditor who has failed to file a proof of claim, the debtor shall provide to the trustee the creditor's telephone number and full account number.
