9004–2 CAPTION — PAPERS, GENERAL The caption shall include the name of the debtor, the case number, the chapter under which the case is filed, the adversary proceeding number, if any, and the name of the judge to whom the case is assigned. The caption also shall contain a specific statement of the nature of the pleading or other paper and the specific relief requested or to be granted. The caption shall be phrased so that the relationship to prior filings or other papers, if any, is explicit. A filing or other paper, including a proposed order, shall clearly identify any other filing or paper to which it relates, if any, by reference to the docket number.
