9011–2 PRO SE PARTIES
(a) Individual Debtor. Unless otherwise ordered by the court, an individual appearing pro se shall include with his or her initial filing the individual's current mailing address, and domicile address, if different from the mailing address, including zip code, and telephone number. In the event of any change in the mailing address or telephone number of the individual appearing pro se, such individual shall file and serve on all creditors and parties in interest a notice of change of mailing address or telephone number within fourteen (14) days from the date of such change.
(b) Non-Individual Entities. A corporation, partnership or entity other than an individual shall not appear pro se, except as provided by Rule 9010(a).
