RULE 1005-1 CAPTIONS The caption of Documents filed with the Court should comply with the appropriate Official Form(s). Every case and adversary proceeding number must be followed by the letters which designate the Judge to whom it is currently assigned. The Chapter under which a case proceeds should be shown below the case number.
