LR 3-1. CIVIL COVER SHEET When a civil action is filed in this court, the plaintiff must file a completed civil cover sheet on the form prescribed by the Administrative Office of the United States Courts (AO Form 44). The civil cover sheet is for statistical purposes only and does not affect the legal rights of the parties.
