LSR 1-2. INMATES: ADDITIONAL REQUIREMENTS FOR APPLICATIONS TO PROCEED IN FORMA PAUPERIS An inmate seeking to proceed in forma pauperis must submit, in addition to the application form, a certified copy of the trust fund account statement (or institutional equivalent) for the 6-month period immediately preceding the filing of the complaint or petition.
