LSR 1-4. APPLICANT FOR FORMA PAUPERIS STATUS NEED ONLY FILE ORIGINAL COMPLAINT, PETITION, OR MOTION An applicant seeking to proceed in forma pauperis need only file one copy of the complaint, petition, or motion together with the application. Copies required for service will be made by the clerk upon the court's authorization of the application.
