Rule 3.1. Civil Cover Sheet
(a) Civil Cover Sheet Required. Every complaint or other document initiating a civil action shall be accompanied by a completed civil cover sheet on a form available at the Court's website. This requirement is solely for administrative purposes, and information appearing only on the civil cover sheet has no legal effect in the action.
(b) Failure to File Civil Cover Sheet. If the complaint or other document is filed without a completed civil cover sheet, the Clerk of Court shall promptly give notice of the omission to the party filing the document and direct the party to file the missing documentation. Should the outstanding civil cover sheet not be filed within three days, the Court may dismiss the action.
(c) Persons filing civil cases pro se are exempt from the foregoing requirements.
