Local Rule LR3.1: Civil Cover Sheet

LR3.1 Civil Cover Sheet Every complaint or other document initiating a civil action shall be accompanied by a completed civil cover sheet, on a form (JS-44) available from the Clerk. This requirement is solely for administrative purposes, and matters appearing only on the civil cover sheet have no legal effect in the action. If any related case has previously been filed, counsel shall call it to the Court's attention in the "Remarks" portion of the civil cover sheet.
Persons filing civil cases pro se are exempt from this requirement.
