LR 3.03 – CIVIL COVER SHEET The filing attorney must prepare and file a civil cover sheet (Form JS-44) with each civil complaint filed. Civil cover sheets (Form JS-44) and instructions for the preparation thereof are available online and from the Clerk's Office.
