Rule Form 3 Petition for Review of Order of an Agency, Board, Commission or Officer

Form 3 is a template for asking a federal appeals court to review a decision made by a government agency, board, commission, or officer. If you disagree with an official ruling from a government body, this is the form you would use to start that process.

The form is straightforward. You fill in the circuit court, your name as the petitioner, the name of the agency that made the decision, and a description of the order you want the court to review. You also include the date the order was made and the contact information for your attorney.

The asterisk pointing to Rule 15 is a reminder to list all the people or parties joining the petition. If more than one person is challenging the agency's order, all of their names need to be included.
