Rule 8.003 Family Law Cover Sheet

When you start a new juvenile case or reopen an old one in certain parts of Florida's juvenile court, you must fill out and turn in a specific form. That form is called the Cover Sheet for Family Law Cases, and its form number is 12.928.

You hand this form to the clerk at the circuit court when you file your paperwork. The person starting or reopening the case is responsible for making sure this cover sheet is included.
