Rule 94 Form of Oath, Award and Notice of Award

When your case goes through arbitration, there are official forms that must be used. The arbitrators' oath, their final decision (called an award), and the notice telling you about that decision all need to follow specific templates set by the Illinois Supreme Court.

You can find these required forms in the Article I Forms Appendix that goes along with these rules. As long as your documents are close to those templates, they will be acceptable to the court.
