Rule 8.26 Advisory notice to minor

When a minor files a petition asking the court to waive parental notification (meaning the minor wants permission to move forward without telling a parent), the court clerk has to give the minor a specific form called the Advisory Notice to Minor. This form explains important information the minor needs to know about the process.

The clerk also has to keep a record showing that this notice was actually given to the minor. This record gets placed in the court file, so there's proof the minor received the required information.
