A. Registration shall be by affidavit filed with the office of children and family services. That office shall develop and furnish standardized affidavit forms appropriate for effecting the purpose of this Chapter.

A. Registration shall be by affidavit filed with the office of child welfare. That office shall develop and furnish standardized affidavit forms appropriate for effecting the purpose of this Chapter.
B. Registration shall be effective upon filing and shall remain effective until withdrawn by the affiant's written request.
C. The department may establish registration and renewal fees not to exceed twenty-five dollars.

To register under this rule, a person must fill out and file an affidavit, which is basically a signed statement confirming certain facts. Right now, this affidavit goes to the office of children and family services. Starting October 1, 2027, it will instead go to the office of child welfare, since the state is changing which office handles this. Either office is required to create standard forms that people can use to make this process consistent and easy to follow.
Once someone files this affidavit, the registration takes effect right away. It stays active unless the person who filed it sends a written request to cancel or withdraw it. There's no automatic expiration date tied to time alone, it only ends when the person asks to end it.
The department in charge also has the option to charge a fee for registering or renewing the registration. This fee cannot be more than twenty-five dollars. This gives the department some flexibility to cover costs, but it limits how much they can charge people who need to register.
