Rule 60.7 Forms To Be Used for Offender/Prisoner In Forma Pauperis Applications

If you're an offender or prisoner and you want to file a case or appeal without paying court fees, you need to fill out a specific form asking the court for permission. This is called filing "in forma pauperis," which basically means you're telling the court you can't afford the normal filing costs.

Which form you use depends on where your case is going. If you're filing in a district court, including regular lawsuits, PLRA appeals, CARP appeals, or parole revocation appeals, you'll use the form found in Appendix 60.7A. If you're appealing to a Louisiana Court of Appeal instead, you'll need to use the different form found in Appendix 60.7B.

Using the right form matters because it tells the court exactly what type of case you have and where it's headed. Filling out the wrong form could cause delays or problems with your request to waive fees, so it's worth double checking which appendix applies to your situation before you file.
