Rule 152.01 Forms for Small Claims Division

If you want to file or respond to a small claims case, the court has to give you free copies of the forms you need. You do not have to pay for these forms, and you do not need a lawyer to fill them out.

The court provides different forms depending on your situation. There are basic petition forms to start a case, and special forms if the person suing or being sued is a minor (someone under 18). There are also forms for counterclaims, which is when the person being sued wants to sue back, either about the same problem or something different.

Once a case is decided, there are more forms to handle what happens next. This includes forms to collect money you won in a judgment, to show that a debt has been paid off, to cancel or dismiss a case, and to record the court's final decision. All of these forms are free and available at the small claims division of your local circuit court.
