Rule 5.6 Form for Application for Post-Conviction Relief

If you were convicted of a crime and want to ask the court to review your case after the fact, you need to use a specific form to make that request. This is called applying for "post-conviction relief," and it means you're asking the trial court to look at problems with your conviction or sentence that weren't already fixed through a regular appeal.

The form you need is called Form 13.11, and you can find it in Section XIII of the court rules. You file this form with the trial court, which is the same court that originally handled your case.

If your case involves the death penalty, the rules are different. Capital cases follow a separate process laid out in Section IX, so you would need to look there instead of using the standard form.
