If you file a legal document that starts a claim, counterclaim, cross claim, or third-party claim, the title of that document has to include certain required notices depending on the type of case. This helps make sure everyone involved knows right away what kind of case they're dealing with.
If your case involves arbitration, the title needs to follow the rule about arbitration cases (UTCR 13.060). If your case is about water rights, the title needs to follow that specific rule (UTCR 5.090(1)). And if your case involves a claim against a health care provider or health care facility, like a medical malpractice case, the title needs to include the notice required under Oregon law (UTCR 5.090(2), tied to ORS 31.250 and 31.272).
The main point is simple. Depending on what your case is about, you can't just title your document however you want. You need to add specific language so the court and other parties know the case involves one of these special situations from the start.
