Rule 9.330 Guardian’s Report in Minor Guardianships

If you are a guardian for a minor child, you have to send the court a written report every year. This report is due within 30 days after the anniversary of when you were appointed guardian. So if you became the child's guardian on March 1st, your report is due by March 31st each year going forward.

The report has to be signed as a sworn statement, meaning you are confirming under penalty of perjury that everything in it is true. If you happen to be outside the United States when you sign it, you can use a different type of unsworn statement that Oregon law allows instead.

You also need to send copies of this report to certain people connected to the case, as listed in Oregon law (ORS 125.060(3)). These typically include people with a legal interest in the child's wellbeing.

Finally, you should use the standard report form found on the Oregon courts website, unless your local court district has its own required form instead. Check with your local court to see if they have a specific version you need to use.
