Rule 104 Design of Forms

The Court Administrator of Pennsylvania is responsible for creating the official paperwork used in criminal cases. These forms are made with input from the Criminal Procedural Rules Committee, a group that helps shape how criminal cases are handled.

These forms exist to make sure the rules of criminal procedure are actually carried out in courtrooms across the state. When a rule requires something to be written down or filed, there is an official form for it.
