Rule 1165 Design of Forms

The Court Administrator of Pennsylvania is in charge of creating the official forms used in juvenile court cases. These forms are made with input from the Juvenile Court Procedural Rules Committee.

If you need to file paperwork in a juvenile court case, the forms you use come from this process. This helps make sure everyone across Pennsylvania is using the same standard paperwork.
