Plain-English Summary (for reference only — not a substitute for the rule text above)
This summary is not the rule. The rule text controls.
When a minor needs to file an application with the court, there is a specific format they must follow. The Supreme Court has approved standard forms, and the application needs to match those forms closely.
The application also requires a separate unsworn verification, which is a written statement confirming that the information in the application is true. This statement does not need to be sworn before a notary, but it still needs to be filed along with the application.
If the application is being filed for someone who has been legally declared unable to manage their own affairs, extra information will need to be included in the application beyond what the standard form requires.
