Rule 5 Required Coversheet

If you are starting a tax case in Arizona's Tax Court, you must include a special cover sheet when you file your paperwork. This cover sheet is a form chosen by the Superior Court in Maricopa County.

You turn in this completed form to the Tax Court clerk at the same time you begin your case. Make sure the form is fully filled out before you submit it.
