4004-1 Debtor Certification in Support of Chapter 13 Discharge. As soon as practicable after the chapter 13 trustee files a notice of completion of chapter 13 plan payments, the debtor shall file a certificate containing the information required by Code § 1302(d)(1)(C)(ii) and (iii) and Code §§ 1328(a) and (h). Note: Form NM LF 4004-1 was created by the clerk for compliance with this rule.
