4004-2 Debtor Certification in Support of Chapter 12 Discharge. As soon as practicable after completion of chapter 12 plan payments, the debtor shall file certificate containing the information required by Code §§ 1202(c)(1)(C)(ii) and (iii) and Code §§ 1228 (a) and (f). Note: Form NM LF 4004-2 was created for compliance with this rule.
