Rule 8.34 Juvenile Procedure Forms — Judicial Waiver of Parental Notification

This rule provides sample paperwork for a minor who wants to get an abortion without telling a parent. Iowa law normally requires a parent to be notified before a minor can have an abortion, but a minor can ask a court to waive that requirement. This rule lists the forms used in that court process.

The forms cover each step of the process. There's a form to start the case (the petition), a form for the minor to explain her situation using a fake name to protect her privacy, and forms to appoint a lawyer or guardian to help her. There are also forms the judge uses to schedule a hearing, explain her rights to her, and issue a final decision.

The last two forms matter if the court doesn't act fast enough or if the minor disagrees with the decision. One form confirms that the waiver counts as approved because the court missed a deadline. The other lets her appeal if the judge says no.

These forms are examples, not strict requirements. Courts and the people involved can use different wording, but whatever they use has to cover the same basic information as these samples.
