Plain-English Summary (for reference only — not a substitute for the rule text above)
This summary is not the rule. The rule text controls.
If you are serving as a guardian for a minor child, you must use the specific court forms listed in this rule whenever you file paperwork related to the guardianship. You cannot substitute your own forms or documents. These standard forms cover things like your background check, a parent's consent to the guardianship, the oath you take to serve as guardian, your initial plan for caring for the child, your yearly reports on how the child is doing, and your final report when the guardianship ends.
One of these forms, the Background Check Information form, is kept confidential. This means the information you provide about your background is not part of the public court record. This protects your personal information while still letting the court verify that you are a safe and appropriate guardian for the child.
