Cover Sheet for use with Rule 1-145 NMRA

When someone files financial paperwork about a protected person (like someone under a conservatorship or guardianship), this cover sheet gets attached to keep that paperwork private. It tells the court that the documents contain confidential financial statements and need special protection under Rule 1-145.

Once this cover sheet is used, the court automatically seals the financial records. No one has to ask a judge to seal them first, it happens on its own.

The rule also keeps these records away from almost everyone, even people you might expect to have access. This means the protected person themselves, the other parties in the case, the guardian, and even the lawyers involved cannot see these financial statements unless a judge specifically allows it. Only the State Auditor gets automatic access. This tight control exists to protect sensitive financial details about someone who may be vulnerable, like an elderly person or someone with a disability who has a conservator or guardian managing their money.

The form itself just asks for basic details: the case number, the county and district where the case is filed, how many pages are being submitted, and whether the person filing it is the conservator or the guardian. It's a short cover page, but it carries real legal weight because it triggers these strict confidentiality protections the moment it's filed.
