Rule 21 Forms

Traffic tickets in Ohio must follow a standard format. The forms shown in the appendix of the traffic rules are required, meaning courts and police cannot just make up their own versions.

One exception allows agencies to make extra copies of any part of the ticket if they need them. The back of the copy kept by the police or issuing agency can be set up in whatever format that agency chooses.
