Rule 15 Duplicate Certificates

If you lose your certificate, or if it gets damaged or destroyed, you can ask the Board to issue you a duplicate. To do this, you need to send a written request along with a sworn statement (called an affidavit) that explains what happened to your original certificate.

The affidavit needs to give enough detail to prove the loss, damage, or destruction actually happened. Once the Board reviews your request and is satisfied with your proof, they can approve a duplicate certificate for you. This process protects certificate holders who need a replacement without having to go through the entire original certification process again.
