Rule 9.030 Addresses and Telephone Numbers Required

Every document you send to the court must include contact information on the last page. This means your name, address, and phone number need to be typed or printed at the end of whatever you submit, following the format required by court rule 2.010(6).

If you are a guardian, conservator, or personal representative and you submit a proposed order to the court, that order also needs your name, address, and phone number printed on the last page. This helps the court know exactly who is involved and how to reach them.

If your address or phone number changes at any point during the case, you need to tell the court right away. This applies to attorneys, people representing themselves without a lawyer, guardians, conservators, and personal representatives. You cannot just mention the change in another document. You have to send a separate document specifically about the update, so the court's records stay accurate and current.
