The Office of the Judicial Administrator for the Louisiana Supreme Court is in charge of creating the official forms that people need to use under this rule. If you need a specific form for a court process covered by this rule, that office is the source for it.

The rule also notes a change from the past. Two older parts of this rule, known as Sections 7 and 8, were removed completely. This change took effect on February 27, 2014. If you are looking at older versions of the rule or older case documents, you may see references to those sections, but they no longer apply.
