If someone wants to file for post-conviction relief in Louisiana, they have to use a specific standard form. This form is called the Uniform Application for Post-Conviction Relief, and it's included as Appendix A in the Louisiana Supreme Court's rules.

This requirement has been in place since January 16, 1981. Anyone filing this type of request after that date must use this exact form. This means people can't create their own version or use a different format when asking the court to review their conviction after their case is already final.
